@Spike-from-NH: Yup, I noticed some of the other things you commented on, but wasn't sure what the right thing was to do there.
In most cases, I don't think pipelining/superscalarity/out-of-orderness makes a difference to the program's view of the PC, otherwise procedure calls get a bit complicated. There might be some imprecision about the instruction that caused an exception; for example, in the IBM System/360 Model 91, several exceptions, such as overflows, underflows, and zero-divide exceptions, did not indicate the instruction that got the exception.[1]: 5, 14  But, yeah, that gets a bit tangled up in the weeds.
