สัจนิยมเหตุซึมเศร้า

ทฤษฎี สัจนิยมเหตุซึมเศร้า (อังกฤษ: Depressive realism) เป็นสมมติฐาน^[1] ว่าบุคคลที่รู้สึกซึมเศร้าทำการอนุมานที่ตรงกับความจริงมากกว่าบุคคลที่ไม่รู้สึกซึมเศร้า แม้จะเชื่อกันโดยทั่วไปว่า บุคคลที่มีภาวะซึมเศร้าจะมีความเอนเอียงทางประชานแบบลบ (negative cognitive bias) ที่มีผลเป็นความคิดเชิงลบอัตโนมัติที่ปรากฏบ่อย ๆ พฤติกรรมที่ไม่เหมาะสม และความเชื่อผิดปกติเกี่ยวกับโลก^[2][3][4] แต่สมมติฐานนี้เสนอว่า ความคิดเชิงลบเหล่านี้ อาจจะสะท้อนการประเมินความจริงเกี่ยวกับโลกที่ถูกต้องแม่นยำกว่า และว่าบุคคลที่ไม่มีภาวะซึมเศร้ามีการประเมินความจริงที่เอนเอียงไปทางบวก^[5] ทฤษฎีนี้ยังเป็นเรื่องที่ถกเถียงกันในระดับสูง เพราะว่า ถ้าเป็นจริงแล้ว กลไกทางประสาทที่การบําบัดทางความคิดและพฤติกรรม (cognitive behavioral therapy) สำหรับโรคซึมเศร้า ทำการเปลี่ยนแปลง ก็กลายเป็นเรื่องที่ไม่ชัดเจน^[6] แม้ว่าจะมีหลักฐานที่สนับสนุนความถูกต้องของทฤษฎีนี้ ปรากฏการณ์นี้อาจจะจำกัดอยู่ในเหตุการณ์เพียงไม่กี่อย่างเท่านั้น^[7]

หลักฐานสนับสนุน

มีหลักฐานสนับสนุนทฤษฎีนี้โดยประการต่าง ๆ รวมทั้ง

• ในการทดลองเกี่ยวกับการแปลสิ่งเร้าผิดว่าควบคุมได้ ผู้ร่วมการทดลองจะทำการกดปุ่มไฟ โดยมีไฟเปิดปิดที่ไม่ได้ขึ้นอยู่กับปุ่มกด เมื่อมีการให้ผู้ร่วมการทดลองกดปุ่ม และให้คะแนนว่าตนเองสามารถควบคุมการเปิดปิดของไฟได้เท่าไร ผู้มีภาวะซึมเศร้าให้คะแนนที่ถูกต้องมากกว่ากลุ่มควบคุมที่ไม่มีภาวะซึมเศร้า^[8]
• ส่วนอีกงานทดลองหนึ่ง ที่ให้ผู้ร่วมการทดลองทำงานอย่างหนึ่ง แล้วให้คะแนนผลงานของตนโดยไม่ได้รับความคิดเห็นอะไรจากผู้ทำการทดลอง ผู้มีภาวะซึมเศร้าให้คะแนนแก่ตนเองเหมาะสมกับผลงานดีกว่าผู้ไม่มี^{[9][10][11][12]}
• ส่วนในอีกการทดลองหนึ่งที่ผู้ร่วมการทดลองทำงานหลายอย่างเป็นชุด โดยที่ผู้ทำการทดลองมีการให้ความคิดเห็นเกี่ยวกับผลงานเมื่อเสร็จงานแต่ละอย่าง และผู้ร่วมการทดลองจะให้คะแนนตนเองโดยองค์รวมเมื่องานทั้งหมดเสร็จแล้ว ผู้มีภาวะซึมเศร้าก็ยังให้คะแนนตนเองได้แม่นยำถูกต้องกว่าผู้ไม่มีภาวะซึมเศร้า^{[13][14][15][16][17][18]}
• เมื่อมีการให้คะแนนตนเองทันทีหลังจากทำงานอย่างหนึ่งเสร็จ หรือว่า รอสักพักหนึ่งแล้วค่อยให้คะแนน ผู้มีภาวะซึมเศร้าก็ยังให้คะแนนตนเองถูกต้องแม่นยำในทั้งสองกรณีมากกว่าผู้ไม่มี^[19]

โดยที่สุดแม้ในการทดลองที่เช็คการทำงานของสมองโดย fMRI คนไข้ภาวะซึมเศร้าก็แจ้งความที่ตนเป็นเหตุก่อให้เกิดเหตุการณ์ทางสังคมทั้งที่เป็นเชิงลบและเชิงบวก ได้แม่นยำกว่าคนปกติที่ไม่มีภาวะซึมเศร้าผู้มักจะมีความเอนเอียงเชิงบวก^[20] ความแตกต่างของการทำงานในสมองนั้นเห็นที่

• เครือข่ายสมองกลีบหน้า-กลีบขมับ มีระดับการทำงานที่สูงกว่าในสองกรณี คือ
  • ในผู้ที่ไม่มีภาวะซึมเศร้า เมื่อบอกเหตุ (ที่ก่อเหตุการณ์) โดยที่ไม่ได้เข้าข้างตนเอง
  • ในผู้มีภาวะซึมเศร้า เมื่อบอกเหตุ แม้ที่เข้าข้างตนเอง
• การทำงานเชื่อมต่อกันระหว่าง dorsomedial prefrontal cortex seed region และระบบลิมบิก ที่ลดลง เมื่อคนไข้ภาวะซึมเศร้าบอกเหตุเข้าข้างตนเอง

หลักฐานคัดค้าน

มีหลักฐานต่าง ๆ ที่คัดค้านแนวคิดบางอย่างในทฤษฎีนี้รวมทั้ง

• เมื่อมีการให้คะแนนกับผลงานทั้งของตนและของผู้อื่น ผู้ไม่มีภาวะซึมเศร้ามีความเอนเอียงเชิงบวกเกี่ยวกับตน แต่ไม่มีความเอนเอียงเมื่อให้คะแนนผู้อื่น ในนัยตรงกันข้ามกัน ผู้มีภาวะซึมเศร้าไม่มีความเอนเอียงเมื่อให้คะแนนตนเอง แต่มีความเอนเอียงเชิงบวกเมื่อให้คะแนนผู้อื่น^[21][22][23]
• เมื่อมีการประเมินความคิดของตนโดยเป็นสาธารณะหรือโดยเป็นส่วนตัว ผู้ไม่มีภาวะซึมเศร้ามีการมองในแง่ดีในความคิดที่เป็นสาธารณะมากกว่าที่เป็นส่วนตัว ส่วนผู้มีภาวะซึมเศร้ามีนัยตรงกันข้าม^{[24][25][26][27]}
• เมื่อมีการให้คะแนนกับผลงานของตนทันทีหรือรอเวลาสักพักหนึ่ง ผู้มีภาวะซึมเศร้าแม่นเมื่อให้คะแนนทันที แต่ว่าโน้มไปทางเชิงลบเมื่อรอเวลาสักพักหนึ่ง ส่วนผู้ไม่มีภาวะซึมเศร้าเข้าข้างตนเองทั้งทันทีและรอเวลาสักพักหนึ่ง^[28][29]
• แม้ว่าผู้มีภาวะซึมเศร้าจะมีการตัดสินใจที่แม่นยำกว่าว่า ตนเองไม่สามารถควบคุมสถานการณ์ได้ เมื่อตนไม่สามารถควบคุมได้จริง ๆ แต่ว่า ความรู้สึกว่าควบคุมไม่ได้ก็ยังเกิดขึ้นในสถานการณ์ที่ตนเองจริง ๆ แล้วสามารถควบคุมได้ ซึ่งแสดงว่า ทัศนวิสัยของผู้มีภาวะซึมเศร้าไม่ได้แม่นยำกว่าโดยรวม ๆ^[30]
• เมื่อทำการศึกษาในสถานการณ์จริง ๆ นอกห้องแล็บ ผู้มีภาวะซึมเศร้ามีความแม่นยำในการประเมินน้อยกว่า และมั่นใจมากเกินไปในคำพยากรณ์ของตนเกี่ยวกับอนาคต มากกว่าผู้ไม่มีภาวะซึมเศร้า^[31]
• การหาเหตุก่อเหตุการณ์ที่แม่นยำของผู้ร่วมการทดลอง อาจจะสัมพันธ์กับสไตล์ในการหาเหตุ (attributional style) โดยทั่ว ๆ ไป ไม่ใช่กับอาการซึมเศร้าที่ตนมีหรือไม่มี^[32]

คำวิจารณ์ต่อหลักฐาน

มีนักวิชาการบางท่านที่เสนอว่า ไม่มีหลักฐานที่สามารถสรุปได้จริง ๆ เพราะว่า ไม่มีมาตรฐานในเรื่อง "ความจริง" ของโลก ดังนั้น ข้อวินิจฉัยเกี่ยวกับการประเมินความจริงไม่น่าเชื่อถือ และผลที่แสดงอาจจะไม่สัมพันธ์กับเหตุการณ์จริง ๆ ในโลก^[33] เพราะว่า งานวิจัยเป็นจำนวนมากมักจะให้ผู้ร่วมการทดลองแจ้งอาการซึมเศร้าด้วยตนเอง (self-report) ดังนั้น คำวินิจฉัยว่ามีภาวะซึมเศร้าหรือไม่อาจจะไม่เป็นจริงเพราะว่า การรายงานถึงตนเองมักจะมีความเอนเอียง ที่จริง ๆ แล้ว ควรจะใช้วิธีที่เป็นกลาง ๆ (เป็นปรวิสัย) อย่างอื่นในการวัดความซึมเศร้า เนื่องจากว่า การออกแบบงานศึกษาเหล่านี้ไม่ได้เลียนแบบเหตุการณ์ที่เกิดขึ้นจริง ๆ ในโลก ความเป็นจริงในโลกของสมมติฐานนี้ก็ยังไม่ชัดเจน^{[โปรดขยายความ]} และยังมีความข้องใจอีกด้วยว่า ปรากฏการณ์นี้อาจเป็นผลข้างเคียงของผู้มีภาวะซึมเศร้า ที่อยู่ในสถานการณ์ที่เผอิญมีความเป็นไปตรงกับความเอนเอียงเชิงลบของตน^[34][35][36]
